Polk County Program for Prescription Assistance

For safety net patients released from the Polk County Jail, there is now a system to provide access to a health provider in conjunction with up to 90 days of behavioral health prescription drug coverage.  The additional 90 days of access to free behavioral health medications will fill a critical gap until the individual can be transitioned to long term behavioral health solutions.

Medications will be dispensed in 30 day quantities with up to two 30 day refills.  There is no cost for the prescribed medications.  To receive your medications, you must see a medical professional at Primary Health Care at 1200 University Avenue, Suite 120 in Des Moines  248-5100

Eligibility:  A safety net patient released from the Polk County Jail in need of assistance to treat a behavioral health disorder must meet the following criteria:

  • Individual or household income at or below 200% of the Federal Poverty Level
  • Lacks health insurance or prescription coverage that would cover prescribed medication
